to use it when your Health is at <50% and so on... Once you're done
with that, slide over to the right where it says (Action) and click on
that... Select Use health poultice: least powerful, since if the potion
used isn't strong enough, the condition will activate again and use
another one... So, you save your stronger potions from being used up
too early...
As for the wheel, I don't use it since I assign
things via the management menu, which gives me more details but you can
toggle the Radial Menu to Toggle open/closed so that you need only tap
the LT to open and close it instead of holding the LT down the whole
time... Which is found in the options menu by pressing Start...
Now,
you may gain extra condition slots via using a point in your Skills tab
towards Combat Tactics... So, all in all, you may have up to 6 slots...

The bold part I did for all the characters and it helped so much. I didn't know how to use that function until yesterday. Now they all seem to kick butt and heal them selves appropriately. Now I haven't played very far as a Mage. I'm guessing staying back and setting your party to be more aggressive would help. Set them to either attack the closest seen enemy or to attack the enemy attacking you.
