FYI: The "default" difficulty is not set to "default"
If some of you were wondering why you couldn't earn any achievements in this game, this might be the reason why. For some reason, Stainless Games set the immediate post-purchase difficulty to "Easy" as the default setting. You cannot earn any achievements at this difficulty. The way to change this is to go into "Help & Options". Notice the "Settings" bar is highlighted. What's not immediately apparent due to its design and appearance is that this "Settings" bar is not a banner or header for settings but an actual selectable button itself. Click your "A" button. After you select it, you will see there are four game difficulty settings: Easy, Default, Hard, and Custom...

Hope this info helps some of you!

No one has this problem because Stainless Games preset everything to default setting.  Missile Command and Centipede and Millipede are set to default from the beginning.  Someone messed around with your settings or you could've been drunk who know's, but it started off at default settings.

Perhaps.

I'd just like to say again how much I hate Throttle Monkey.

I actually had the same problem for the first few hours I played this game, and I could not figure out why I was not earning any achievements.  Then, as you stated, I realized that I needed to go into settings to switch to default.  So, yes, the first poster did make an accurate statement.

I'm sorry...I know it's not normal, but...I can't help it.

It didn't happen to me, but I was talking to someone when it happened to them. They JUST purchased the game and started playing and the achievements weren't working. Turns out, it was set to hard off the bat...
