I've been surfing the web trying how to unlock all the costumes. I've confirmed
Andy's Surf outfit is for getting grades on all Childs play stages
Zack's Deathgame  is for getting grades on all Thriller Stages
Carrie Ninja outfit for getting all of her Level 3  weapons

rumors what I found to be incorrect form online
You do not get an outfit for collecting all level 2 weapons for any character. (which I currently have for all 4 characters)
You do not get an outfit for completing the Bestiary

Anyone has all the costumes, and know how they got them?

for all characters the way to get em is:

1.) Get lvl 3 for all weapons

2.) Get S ranks on every lvl on every difficulty(each character gets a diff.)

3.) Beat that diff of the game to get a costume(each chara. gets a diff. do not need an S rank)

So...you get the costumes for getting levels 1, 2 and 3 of weapons? because I have levels 1 and 2 and I have nothing to pick.

Only lvl 3 weapons, finish a difficulty, and all "S"s on a difficultly will unlock costumes

Well, I know most of the costumes except grade S ones.

Zack:
Deathgame: Complete thriller or higher
The One(matrix outfit): All level 3 weapons
Cowboy?(I don't have): Thriller or higher all S rank

Andy:
Surfer: Child's play or higher
Soldier: All level 3 weapons
Scout (like club scout): All S grades in child's play

Carrie:
Vally Girl (denium clothing, but shaped like her normal outfit): Horror or higher
Ninja: All lvl 3 weapons
Don't know her All S grade on Horror or higher outfit

Jennifer:
Cheerleader: Complete Madness
Nurse: All Lvl 3 weapons
Swimsuit?: I don't have, probably never will.

Oh ho ho i do. I completed this game this morning and it was one hell of a ride. I'll upload a video of each costume of you want, BUT here is the definitive list... most people probably knew this though. You need to... Beat madness difficulty (Each difficulty unlocks a new costume, and its stackable) Upgrade every characters weapon to level 3 (This takes about a playthrough and a half... a total of 6 playthroughs :( ) And the creme de la creme... S rank every level. (Also stackable, good luck doing madness) As I said, I have done this, i'm probably the only one who has (If you dont believe me look at my gamercard... 100% legit) and i recommend you don't try it. Good luck!

theres a few others who have done it as well. i plan on getting going on it again, only a few levels more to s-rank on madness, then ill start working on getting all lvl 3 weapons for the 3 characters i havent done it for. congrats though, its no easy task

I've only seen one other person (who is actually on my friends list) who got this achievement, but he hasn't completed the game yet. (He's at 880 I think) Literally everyone I found on this games leaderboard (via mygamercard.net) with this achievement was a cheater. But if you are going for it, I had the most trouble with levels 4,14, and 18. Grinding for the level 3 weapons took way too long lol. All I can say you'll need to do it is patience, a steady hand, and jolt cola. LOTS of jolt cola.
