I got it playing on very hard mode,you have to just a tiny sliver of health left when you win a round,not really one you can go for,best to just let it happen naturally when you're playing.

You can get this ach. in Versus mode as well, if you wanna do it with a 'friend'.

I remember the time I got that Close Call achievement not too long ago, and I barely see a green sliver on my character's lifebar, but ended winning the round... It's not easy to get it, but by playing a lot, you'll get the chance time by time...
