So I just went through the set list in coop with someone for the second time.  The first time I played through it with me as lead in every song and the achievement unlocked fine.  Now this time I was bass every song, but the achievement did not unlock.  What the hell is going on here?

If you are doing the achievements online, you should know that they are 'glitchy.' Lots of folks have had a hard time doing them with a COOP partner over LIVE.

If you are trying to get the achievements on the same Xbox, rather than over LIVE, you may want to let the player who is going for the achievements sign into the Player one slot, and then the person who is assisting can sign into player 2 slot. Hope that helps.

It was local and I did all of that.  Thanks anyway though.  Anyone have any other suggestions?

This didn't matter when I did it, but did either of you change difficulties throughout the setlist? You never know what might cause some of these achievements to glitch.

Nope.  She is not experienced so when we played with me as lead we did medium for every song and when I was bass we did easy every song.

That's a bummer. Seriously, that would suck.
