I swear I've explored the North like crazy and cannot find the witch elixers, does anyone off hand remember which caves they were in?  I know the last cave before the witch queen has one.

The one that had the big tree in there, that had one.  (i think it was the 2nd or 3rd one)

 

Figure I'd update my topic since i got it:  You don't have to get all three in one level run through as long as you have 3 total in your inventory you will get it.

I only got one witch elixer and got the achievement!

I am up to 4 elixers with no achievement now.

I only have two elixirs and cannot find the third.  I've been from top to bottom in all of the caves.

I do know that, in order for the elixirs to count, you have to defeat the Queen Witch.  Even if you beat her once and have two elixirs, if you find another elixir on a 2nd play-through, you will have to beat her again for it to count.

Theres only 2 witch elixers in the caves, so in order to get 3 you need to replay the level. There is one in the cave with the tree, and the other one is in the cave right before fighting the queen witch. After you beat her, restart that part of the level and just go to the first cave with the tree and it should unlock the moment you pick it up.
