if you like pac-man I would say get it.  The game is classic, you should pretty much no what to expect.

Tackling them in order:

1) The achievements are not too difficult.  You can continue progression from whatever level you made it to, and coupled with permission to adjust total lives and still earn achievements it is merely a time investment to get to the highest level necessary achievement-wise.  The only challenging achievement, in my opinion, is eating all four ghosts with all four power pellets on a given level.  Many feel this is easier in Ms. Pac-Man than in the original Pac-Man, but it's still the hardest achievement in the game.

2) How much better is it than Pac-Man?  I'd say it's slightly better.  The map variance, the moving fruit, the ghosts being a little less obvious in their predictability...this all adds up to it being more varied and challenging.  However, it pales in comparison to Pac-Man C.E.

3) Pac-Man games are gender neutral.  Ms. Pac-Man is no exception.  There is barely any story to these games.  You either like dodging ghosts and eating pellets in a maze or you don't.

4) I believe there are a fair number of people on the leaderboards, but I don't know how active any movement is on it.  I didn't really care about the leaderboard so I didn't look closely.

5) If you like Pac-Man games, the $5 price-tag is pretty reasonable.  You don't get games for less on XBLA (other than community games).  If you only like to play games until you have all the achievements, though, this one is pretty short.  Pac-Man C.E. will take a longer time investment, and while it's $10 I do think it's a vastly superior product.  But, if you ever played Ms. Pac-Man in the arcade, this will give you the nostalgia fix you're looking for, and Ms. Pac-Man remains a good game to this day.
