I remember when I got PSO first I was automatically given 99 items, rare items, etc, by people before I even got into the game.

I have played with a guy who said he had his account hacked and all of his items stolen... whether or not it's true I'm not sure, but I haven't seen anything non legit myself...

I've seen a few reports of that happening, but it's due to their gamertag getting "hacked".

AFAIK the PSU servers and all of it's content are totally safe to this point.

There's ALWAYS a way to cheat in every game.There are people that dedicate their (probably otherwise boring) lives to finding a way around every anti-cheat system ever created. If cheaters can show up in Halo 2, which had the most well funded and dedicated system ever to stop it, they'll show up anywhere. So it's really just "not yet."

but Halo 2 was on the regular xbox, which was 'moddible'

ppl kept there old xboxs so they can still cheat, knoing that the 360 is unable to moddify
